# The Remaining Maximum Is the Number the Treatment Plan Depends On

*Most patients finish the year with benefits unspent. The chart rarely says how much.*

A patient has a $1,500 annual maximum and has used $1,160 of it. A crown is on the schedule. The plan has $340 left to put toward the crown, and the coverage percentage decides how much of the fee that covers. The rest becomes the patient portion.

That arithmetic takes a minute when the remaining maximum is in the chart. When it is not, it takes a call to the carrier, and the call tends to happen at the front desk while the patient is standing there.

| **Only 3.4% of dental patients reach the typical annual maximum, and another 3.3% come within $100 of common maximums such as $1,000 or $1,500.** *ADA Health Policy Institute analysis, 2024, reported in ADA News, December 2025* |

## **Most Patients Finish the Year With Money Left**

For plans on a calendar year, unused benefits generally expire when the year ends. That gives a patient with room left a reason to schedule treatment now, and it gives the practice a reason to know exactly how much room there is.

***Two numbers that measure different things***

A second figure circulates that sounds much higher. CareQuest Institute for Oral Health surveyed adults in 2025 and found that 12% of those with dental insurance reached or exceeded their annual maximum in the past year. The ADA analysis counts dental patients. CareQuest asks insured adults. The two are not interchangeable.

Either way, a full schedule will include some patients close to a limit and many who are not. The chart has to say which is which.

## **The Maximum Is Not the Same Number From Patient to Patient**

The $1,000 maximum has not gone away. The ADA notes that many plans still promote a $1,000 level that was established about 40 years ago. Alongside it sit plans with $1,500, $2,000, and higher.

| **32.8% of in-network annual maximums are between $1,000 and $1,500. Another 48.2% fall between $1,500 and $2,500.** *National Association of Dental Plans data, cited in ADA News, December 2025* |

On a single Friday schedule, that can mean one patient with $1,000, one with $2,000, and one whose plan has a rollover feature. Rollover features exist. Under some of them, a patient who keeps claims below a set threshold can carry part of an unused maximum into the following year, up to a cap. In an example published by Delta Dental of Arkansas in 2022, a member on a $2,500 maximum earns up to $625 in carryover for each qualifying year.

Thresholds, percentages, and caps vary by carrier and by employer group. The question has to be asked for each patient, and the answer has to be written down.

## **What Changes When One Person Owns the Number**

A dental insurance verification virtual assistant from ZIA is a pre-assessed professional who works virtually inside the practice’s own systems, in the same Dentrix, Eaglesoft, or Open Dental as the rest of the team. For remaining maximums, the role records three things for every patient before the visit: the amount left, the deductible status, and the date the plan resets.

The treatment coordinator then plans from the chart. A patient with $340 left gets a conversation about timing while there is still room to plan it. A patient with $1,200 left gets a different one. A patient whose plan carries over part of the unused amount gets a third. Nobody calls a carrier from the front desk while the patient waits.

## **A First Check for the Next Ten Days**

Take the next ten days of the schedule. For each patient with a treatment plan, look at whether the remaining maximum is in the chart. The count of patients where it is missing shows where the work sits today, and who is doing it.
